Important Reminder for Submitting Storage Media to the Center before Destruction!!
Before using this service, please complete the following steps, which are divided into online and physical processes:
Online Process:
For original on-campus information and communication assets, complete the on-campus disposal (impairment)procedure. Ensuring that the Asset Management System of the Assets Department no longer records the asset is necessary.
Please reserve the desired date and time on the Google Calendar (in the below section).
Please read the Disclaimer before signing the Disclaimer Form.
Physical Process:
For original on-campus information and communication assets, fill out the ISMS-P-011-05 Media Destruction Operation Processing Form V2.0. Please print and sign the form (physical document) and return it to the center on the day of media destruction.
Other Precautions Reminder:
Before submitting to the center for destruction, please remove the Hard Drive Rack, various casings, or heat sinks in advance. We only accept the destruction of the media itself (as shown in Figure 2 and Figure 4 below).

We only accept the destruction of the media itself (as shown in Figure 2 and Figure 4 below). After the hard drive is destroyed, the fragmented parts may cut, puncture the skin, and create debris. Please prepare a box or thick bag to store and carry the waste back to avoid injury during transportation (this center does not provide waste cleaning services)."

Service Reservation Instructions
Service Hours: Every Tuesday from 10:00 AM to 11:30 AM
Each appointment slot is 30 minutes long, and can accommodate the destruction of approximately 30 hard drives. If there are more than 30 hard drives to be processed, please book two consecutive time slots!
